 Individuals with an intellectual or developmental disability who need support with daily living, community involvement, or personal care may be eligible. Services are available to residents of Philadelphia, Chester, Delaware, Montgomery, and Bucks counties. 

To find out if you or a loved one qualifies for services:

1. Contact Your County ID/A Office – Find your local office.
2. Request an Assessment – A formal evaluation determines eligibility for IDD or autism services.
3. Provide Diagnosis Documentation – Proof of intellectual disability, autism, or developmental disability is required.
4. Apply for Waiver Services – Medicaid waivers cover many support programs.
5. Get Support Coordination – A Supports Coordinator can guide you through the process.

They act as a key resource for navigating the system and connecting with community resources.
